YourCare Brevard is a Family Care Program for Seniors.  Our purpose is to manage the care of our Brevard seniors allowing them to remain in their home safely and avoid institutionalization.

Eligibility for this program is determined by the Department of Elder Affairs and Department of Children and Families. 

Eligibility requirements include:

      * Age 65 years or older
      * Have Medicare parts A & B or a Medicare HMO
      * Reside in Brevard County
      * Meet physical requirements as determined by the state
      * Meet Florida financial requirements
